Your Hope Center’s Behavioral Health Programs
Your Hope Center provides comprehensive services to address behavioral health needs across all ages. Crisis Co-Response Teams work in the field, including in schools and hospitals. The Community Stabilization Program provides support for individuals transitioning to outpatient care. And School-Based Clinicians offer immediate, front-line behavioral health care to children and their families.
Crisis Co-Response: Immediate Intervention When Needed Most
The Your Hope Center Crisis Co-Response Team is available 24/7 for in-the-moment support to those experiencing a mental health crisis. They provide in-person evaluations, personalized treatment plans, and referrals for continued support. The team can also arrange transportation to medical facilities for the most critical cases, ensuring safety through M1 holds (a 72hour inpatient stay) when necessary. While the number of crisis calls has more than doubled (1,370 calls in 2023/24 compared to 500 in 2018), Your Hope Center has decreased the number of M1 holds (47 in 2022/23 compared to 120 in 2018), saving patients and our community millions of dollars.
Community Stabilization Program: Providing Transitional Support
The Community Stabilization Program (CSP) offers intensive support for high-risk individuals transitioning from crisis or hospitalization. Bilingual clinicians provide crucial care for 8 – 10 weeks, ensuring a safe and smooth transition to long-term outpatient care. Case managers act as navigators, connecting clients with essential social services such as food assistance, substance abuse treatment, housing, childcare, transportation, and more.
Between November 1, 2021, and June 30, 2024, CSP enrolled over 800 individuals, of whom 21% were under age 18. In 2022, the State of Colorado tasked Your Hope Center with delivering Assertive Community Treatment (ACT) programming to provide perpetual care for those suffering from severe and persistent mental illnesses such as schizophrenia. The Community Stabilization Program now supports 21 ACT clients in addition to approximately 35 CSP clients a month.
School-Based Clinicians: Intervention for Students
Through a partnership with the Eagle County School District (ECSD), Your Hope Center has licensed clinicians in every K-12 ECSD school, providing access to mental health resources for students. Recognizing the needs of the diverse community, the program has bilingual clinicians in schools with high concentrations of Spanish-speaking students.
The program’s impact has shown positive results. During the 2023/24 school year, in-school clinicians provided over 17,200 sessions across more than 1,000 students—15% of the ECSD student population. During the 2024/25 school year, Your Hope Center now has seven bilingual clinicians available to students within the district.
“We have been able to place licensed clinicians in every K-12 ECSD school, providing mental health resources to approximately 6,500 students,” says Executive Director Carrie Benway.
A Collaborative Approach
The collaboration between Your Hope Center’s Crisis Co-response Team, Community Stabilization Program, and school-based clinicians supports the community’s mental health needs, including students and families. For example, during the 2023/24 school year, the school-based clinicians received assistance from the Crisis Co-response Team for 35 crisis evaluations in schools resulting in 3 M1 holds for children under 18.
