Removing Barriers and Rebuilding Lives Through Case Management

This year marked an important new chapter for behavioral health services at Vail Health. With the opening of the Precourt Healing Center in May 2025, Case Management expanded beyond outpatient support to include inpatient behavioral health services, creating greater continuity of care for patients and families. Inpatient Case Managers work closely with patients, their families, and clinical teams to remove barriers to care and develop personalized discharge plans. By connecting patients with housing assistance, transportation, and community resources, they help ensure a smooth transition to outpatient services and ongoing support after patients leave the Precourt Healing Center.
“Case Managers play a vital role in helping people navigate complex health care and social service systems,” said Rosanna Mendez, Behavioral Health Case Manager at Vail Health. “For many patients, especially those living with mental health conditions, having a Case Manager means not feeling alone in their journey.”
Navigating a Path Forward
For several years, one Eagle County resident experienced chronic homelessness while living with schizophrenia and other significant behavioral health challenges. With limited resources and a severe condition, he frequently cycled through periods of instability. At times, he ended up in jail—one of the few places where he could access food and shelter during harsh weather.
Through consistent engagement, Case Managers built trust and coordinated critical support, including medication management, behavioral health appointments, temporary shelter assistance, and access to food and hygiene resources. Even when he struggled to stay engaged, the team kept searching for long-term solutions. Working alongside community partners and relatives, the team ultimately helped reunite him with his family and connect him to a more stable support system. That support opened the door to ongoing care and a better quality of life.
Case Management by the Numbers
Thanks to philanthropy, Vail Health’s Case Management team reached more people than ever in 2025
- 512 outpatient referrals received
- 32 active cases per outpatient Case Manager, on average
- 7 inpatient Case Managers serving patients at the Precourt Healing Center

Across all services, every patient receives one-on-one support, including personalized help with treatment goals, care coordination, resource connections, and transition planning tailored to their needs.
Work That Comes From the Heart
For Rosanna, the work is rooted in community.
“I can make a difference in the lives of people in my community, which fills my heart with joy,” she said. “Watching people rebuild their lives and achieve their goals is priceless. I wouldn’t trade that experience for anything.”
