Case Management Services Improve Access to Behavioral Health Care

Case Management Services

Behavioral Health Case Management Services help individuals and families overcome barriers to accessing care. Since October 2021, our dedicated case managers have guided people to available services in the region. The bilingual and bicultural team connects underserved individuals of all ages and families to care and support. These services are either covered by Medicaid or free for those without coverage. In 2024, the team coordinated 3,355 visits, a 141% increase from the previous year, demonstrating a growing impact in the community.

Through Your Hope Center’s Community Stabilization Program, we offer a range of services, from basic navigation and peer support to long-term managed care. Services are available at all Vail Health Behavioral Health clinic locations, and case managers can meet with patients in the community. This multi-tiered approach provides a comprehensive wrap-around of behavioral health and social determinants of health services for Eagle County residents, including:

Personalized Support: Case managers assess patients to determine the right level of care. They create care plans, refer patients to community resources, and follow up to ensure adequate care. By coordinating care across various systems, specialists, and organizations, they ensure that patients receive comprehensive support tailored to their needs.

Expanding Access to Therapy: Case managers enhance access to therapy through referrals and direct support. They link clients to clinical treatment and other services, providing intensive support for high-need cases and ensuring immediate assistance when needed. This approach helps maintain specialized clinical capacity for acute needs while delivering appropriate care at all levels.

Outreach and Education: Case managers and partners at the Edwards Community Health Campus offer mental health and substance misuse education. They participate in community events to reduce stigma and increase access to care. By engaging directly with individuals and families, they help raise awareness and provide valuable resources.
